Insurance Coverage for Physicals/Well-Child Visits
If you are not eligible for a well-child visit within our allowed time frame due to insurance restrictions, we suggest your contact your insurance provider. They might waive this restriction for the purpose of a college entrance physical.
Late submissions
Late submission may result in your inability to participate in team activities upon your arrival to campus, including conditioning, weight training, team and individual practices, testing, and competition.
