Nico, the first time it happened to me it scared the living poo out of me, I hit one, then thought I was standing too close then moved 2 inches back then did the same again, 3,4,5,6 and on the 7th consecutive time I covered a ball in drywipe ink from a pen in my practice bag, hey presto off the toe, I honestly had no clue it was coming off the toe and not the hozzel.
Now I can tell where it makes contact (as you mentioned), it ususally happens on the practice range when Im experimenting with hooks and low shots.
